EBD Alert - Bariatric Surgery Pilot Program Open
Enrollment for the Bariatric Surgery Pilot Program is now open. Eligible employees have two ways to enroll in the program.
Employees who would like to enroll online can do so by logging in to their ARBenefits Member Portal account at www.ARBenefits.org. After logging in to their account, employees can click the “Forms” tab to complete the online enrollment form.
If an employee would like to complete the enrollment process over the phone, they can contact EBD Member Services at 1-877-815-1017 x1, or call Health Advantage at 1-800-482-8416.
