  EBD Alert – November 1, 2018 
  Extension for Submission of Wellness Requirements

  Health Insurance Representatives (HIR): Open enrollment is our busiest time of year.  We apologize for some of the access issues members experienced while trying to submit information.  

  Because we value each member, we are going to allow more time to meet the requirements necessary to receive the wellness discount.  

  EBD has authorized the following:
  1. PCP forms will be accepted until midnight November 15, 2018, so long as the tests were performed on or before October 31, 2018. 

  PCP Forms can be submitted to the fax number on the form, uploaded to the member’s ARBenefits account or e-mailed to AskEBD@dfa.arkansas.gov; 

  2. The online health risk assessment (HRA) will be available until midnight November 15, 2018;

  3. The online tobacco cessation program will be available until midnight November 15, 2018.
