GOLD STAR FAMILY LICENSE PLATE

Eligibility: 

Under Act 493, the applicant must be the spouse, sibling, child or parent of a member of the armed forces of the United States, killed in a conflict recognized by the United States Department of Defense, and who has received a Gold Star Lapel Button issued by the United States Department of Defense.

Requirements: 

The applicant must present a copy of the Report of Casualty DD1300 form.  If spouse, the applicant must present a copy of marriage certificate, or if parent, a copy of birth certificate.  Eligible applicant must be listed as owner or co-owner on the certificate of title of the motor vehicle to which the Gold Star Family plate will be issued.   Under Act 493, evidence of the receipt of a Gold Star Lapel Button issued by the United States Department of Defense under 10 U.S.C. & 1126.  A copy of the final Leave and Earnings Statement issued to the deceased service member showing that he or she was entitled to; Hardship duty pay, Hostile fire pay or Imminent danger pay.
